Types of Mandated Reporters
Everyone should report all observed, known or suspected incidents of adult abuse, but the following persons are required by law to report:
"Any person who has assumed full or intermittent responsibility for care or custody of an elder or dependent adult, whether or not that person receives compensation, including administrators, supervisors, and any licensed staff of a public or private facility that provides care or services for elder or dependent adults, or any elder or dependent adult care custodian, health practitioner, clergy member, or employee of a county adult protective services agency or a local law enforcement agency is a mandated report."
"Any mandated reporter who, in his or her professional capacity, or within the scope of his or her employment, has observed or has knowledge of an incident that reasonably appears to be physical abuse, abandonment, abduction, isolation, financial abuse, or neglect, or is told by an elder or dependent adult that he or she has experienced behavior, including an act or omission, constituting physical abuse, abandonment, abduction, isolation, financial abuse, or neglect, or reasonably suspects that abuse shall report the known or suspected instance of abuse by telephone immediately or as soon as practically possible, and by written report sent with two working days."

Mandated reporting parties come from, but are not limited to, the following categories:
- Health care practitioners, e.g. doctors, dentists, nurses, therapists, and their office staff
- Adult personal care providers, e.g. attendants, day care staff, senior center staff
- Law Enforcement Officers
- Medical Examiners
- Paramedics and firemen
- Code enforcement and animal control workers
- Clergy
- Adult/Child Protective Services staff
- All staff of Orange County Social Services Agency having client contact
- After Jan. 1, 2007, employees of financial institutions
Step by Step Mandated Reporting
Step one: Make a verbal report by telephone as soon as practically possible to Adult Protective Services at 800-451-5155. If the abuse occurred in a long-term care facility call the Long-term Care Ombudsman at 714-479-0107.
Step two: Complete state form SOC 341 (which can be downloaded from this site), Report of Suspected Dependent Adult Abuse in duplicate (or Xerox).
Step three: Mail (you may fax) the original copy of the written report within 2 working days to:

Step four: The second copy of the written report is to be filed according to the reporting party's agency policies and procedures or patient' record manual.
